Preauthorization is required for all non-emergency hospital admissions or within 48 hours following an emergency admission. Certain procedures may require a second surgical opinion or surgery on an out-patient basis.

PPO Hospital in-patient and out-patient services in Anchorage are provided by Alaska Regional Hospital. If you do not use this PPO facility financial penalties will be assessed.
